Social Worker - Whole Health Director

Duties and responsibilities included, but are not limited to, the following:
Coordinate development and implementation of the patient-centered care concept throughout the organization. Guide staff in the application of patient-centered care concepts. Chairperson for the Whole Health and Wellness H&PCC committee. Develop clinics and systems (including documentation templates) required for the delivery of Whole Health Veteran services to include, but not limited to), coaching and veteran education courses. Lead, facilitate, and evaluate system changes that support meeting performance measure goals and effective patient outcomes. Facilitate process improvement in programs/systems that enhance healthcare delivery; analyzes program/system outcomes and plans strategies to initiate needed change. Act as a consultant in patient-centered care principles and performance measures. Effectively use group process, interpersonal, and communication skills to facilitate win-win outcomes in a positive and productive working environment. Use knowledge, expertise, principles or models, and current research to plan, implement, and evaluate organizational progress towards accomplishing patient-centered care initiatives. Use data trends and patterns which indicate need for change. Serve as the liaison between the medical center, patients and staff, and the community regarding the patient experience, patient rights and advocacy. Lead the Whole Health Information Center supporting the center to provide robust Whole Health Veteran Education and Coaching services. Provide annual whole health business plan and program reports. Develop budget requests and manage Whole Health resources. Ensure inventory procedures and logistical procedures for all Whole Health supplies. Utilize political savvy communication and build relationships with members of the Whole Health team including VA Central Office and VISN 19 Whole Health Leadership. Citizenship. Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. GS-12 Social Worker Program Coordinator Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others. Position requires walking throughout the clinic and the Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. Position may require minimal travel into the community to meet with community agencies and partners who assist and support to the Caregiver Support program, its Veterans and caregivers. Must be able to operate a government motor vehicle. This position also requires close visual acuity to perform an activity such as:
preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
