PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes.
HopeWest PACE provides home care, access to the day center and clinic, transportation to and from medical appointments, and activities to enhance the lives of patients and their families.
This innovative program balances medical, emotional, physical, and social needs of seniors.
Essential Duties of the Social Worker:
Completes initial and ongoing assessments of participants Provides interventions such as contact and education with participant and their families, assessments, counseling, mobilizing resources, assessing mental health and advocacy to ensure needs are addressed.
Collaborates with the interdisciplinary team to create and implement individualized plan of cares to meet each participants needs What You Bring:
Masters degree in Social Work from an accredited social work program Minimum of one year experience working with with elderly Excellent communication skills Valid Colorado driver's license, adequate automobile insurance and access to reliable transportation if working in the community setting Ability to work collaboratively and communicate effectively What We Offer:
